Dr. Andy Yablonski (Alex O'Loughlin, ``Moonlight'') leads an elite team of medical professionals who work with organ transplant patients and their families, offering a last chance at hope for many. At the preeminent transplant hospital in the country, Yablonski and his colleagues race against time to save lives, as well as managing the fears and concerns of worried family members of donors and recipients alike.
